Click on a link to the left to display a list of available forms. The forms may be filled out online. To correctly save a completed form, you must click on the 'Diskette' icon (ex.: ) on the Adobe toolbar, and save the file to a desired location on your hard drive. An example of the Adobe toolbar appears below (a pointer is provided below to direct your attention to the 'Diskette' icon):

DO NOT click 'File', 'Save' in your browser's drop-down list - it will not save the form.
