An individual
appearing pro se shall file with such individual's initial notice or pleading
a designation of the individual's residence address and mailing address,
if different, and telephone number where the party can be reached during
daytime hours. An individual appearing pro se who is not a resident of
the Northern District of New York may be required by the court to designate
a mailing address within the Northern District of New York. This requirement
shall not apply to an individual who has appeared solely for the purpose
of filing a proof of claim or interest.
